Review the bill
- Request an itemized bill from the provider’s billing department and compare the procedure codes with the services you received.
- Request an EOB from your insurer; compare the bill with the Patient Balance amount and your medical records.
- When you see a problem, call the billing department; note the name of the person you speak with and the reference number.
Appeal the insurance decision
- Complete the insurer’s form or send a letter containing your name, claim number, and insurance ID number.
- Include documents supporting your appeal, such as a doctor’s letter, with your application.
- If the internal appeal is denied, request an independent external review within 4 months after the final denial notice.
Explore payment and assistance options
- If you are uninsured or do not use your insurance, request a written Good Faith Estimate before planned care.
- Start a dispute if a single provider’s bill is at least $400 higher than the estimate and the other requirements are met.
- Request a payment plan or discount from the provider, and apply for financial assistance from a nonprofit hospital.
